“Effect of different pillow designs on promoting sleep comfort, quality and spinal alignment: A systematic review.”
This review concluded that the aim of a pillow should be to maintain cervical spinal alignment, preserve the natural cervical curve, improve comfort and reduce neck pain and waking symptoms. Stronger clinical trials are still needed, but the direction is clear.
